(Tufts Now/ Springer) — July 1, 2026: A newly published scientific review has raised fresh questions about the long-term health effects of artificial and other non-nutritive sweeteners, suggesting that some sugar substitutes may influence gut bacteria and metabolic health. However, researchers caution that additional studies are needed before firm conclusions can be reached.
The review and meta-analysis, conducted by researchers at the Food is Medicine Institute at Tufts University and published in the journal Current Atherosclerosis Reports, analyzed evidence from 21 randomized clinical trials involving adults. The findings suggest that certain low-calorie sweeteners were associated with increases in fasting insulin and HbA1c, a marker commonly used to measure long-term blood sugar control. Researchers also observed a trend toward reduced insulin sensitivity.

The study examined artificial and other non-nutritive sweeteners by comparing them with non-caloric alternatives such as water or placebo. According to the researchers, this approach helped isolate the physiological effects of the sweeteners themselves rather than comparing them with sugar-containing products.
Lead author Meng Wang, Research Assistant Professor at Tufts University’s Friedman School of Nutrition Science and Policy, said, “What makes our analysis notable is that by focusing on non-caloric comparators, we better isolated the direct physiological effects of the sweeteners themselves, not the calories they replace.” He added, “When pooling findings from individual trials, we see signals that these compounds may have metabolic harms.”

Gut Microbiome May Play a Role
Researchers believe one possible explanation involves the gut microbiome. Since non-nutritive sweeteners pass through the digestive system, they come into direct contact with intestinal microbes. The review highlighted previous experimental work indicating that certain sweeteners may alter both the composition and function of gut bacteria, potentially affecting metabolic processes.
The researchers also reviewed large observational studies that generally found an association between consumption of non-nutritive sweeteners and a higher likelihood of cardiometabolic diseases. However, they emphasized that these studies cannot prove cause and effect because individuals already at greater risk of diabetes or cardiovascular disease may be more likely to consume sugar substitutes. Different sweeteners may also produce different biological effects, making broad conclusions more difficult.

Senior author Dr. Dariush Mozaffarian, cardiologist and director of the Food is Medicine Institute, advised a balanced interpretation of the findings.
He said, “The rapidly increasing use of these sweeteners has outpaced our understanding of their long-term health effects.” He further noted, “If you’re replacing large amounts of added sugar in your diet, such as in multiple servings of soda, these low-calorie sweeteners may be a better alternative. But we can’t simply assume they are safe and innocuous, and avoiding them whenever possible appears a prudent choice.”

The review also identified a challenge facing researchers in the United States: food manufacturers are required to disclose the presence of non-nutritive sweeteners in ingredient lists but are generally not required to specify the quantity used. According to the authors, this limits scientists’ ability to accurately estimate long-term exposure in large population studies.
Systemic Effects of AS on Human Health.
The researchers concluded that more carefully designed randomized controlled trials are needed to better understand how individual sweeteners affect metabolism, cardiovascular health and the gut microbiome over time. They stressed that the current evidence points to possible concerns but is not sufficient to establish that artificial sweeteners directly cause metabolic disease.
